  5. All LVT Corp high-speed Internet accounts come with one email address. Additional email addresses are $1.00 per address per month.

Other Important Information:

  1. Customer needs Ethernet or USB port on their computer. (Windows 95,98, 2000, ME, XP or NT 4.0) Customer is responsible for this equipment.
  2. LVT Corp. does not provide a firewall for your computer(s). The DSL service is "always on", so your computer is open to the outside world. Customer is responsible for providing their own firewall.
  3. These prices do not include telephone service.
  4. Sales Tax is not included in the prices stated above.
  5. When purchasing a DSL modem from LVT Corp, customer is provided a 30 day warranty from LVT Corp. After LVT Corp 30 day warranty expires, the manufacturer warranty is effective.
  6. Subsequent visits to the customer's premises will incur a trip charge ($20.00) and labor charges ($40.00/hour).
  7. Static IP Addresses - $5.00 per IP Address per month.
  8. Separate fees for material such as wiring and telephone jacks may be charged to the customer's account.

Billing information and other fees

  1. Billing begins 30 days from the day the service is activated. Your first bill may reflect prorated service and one full month of service.
  2. Reconnection of service for non-payment - $11
  3. Service/record change - $11

LVT Corp does not supply a firewall. You will be responsible for supplying your own firewall to protect your computer.
